Insurance and Billing

Dental Insurance

We are an out of network provider. We do not accept insurance as a form of payment. As a courtesy, we will submit claims on our patients' behalf. Any/all reimbursements are payable to the subscriber/patient. We will gladly submit for a predetermination of benefits so that you will know what reimbursement to anticipate for your treatment.

 

Our staff will provide you with the necessary forms for insurance reimbursement if you would prefer us not to submit on your behalf. We usually file any pre-estimates, since the insurance company generally requires X-rays, periodontal charting, and a detailed treatment plan. Completed treatment forms are typically submitted after your Estimate of Benefits has been returned to us and treatment has been performed. Questions regarding the terms of your coverage or delays in payment should be directed to your insurance carrier by you or your employee benefits administrator.

 

It is important to remember that your carrier's contract is with you or your employer, not with our office, so we can not guarantee your coverage or benefits.

Please keep in mind that our office will always come under the category of "outside the network" or "non-participating providers."

It is difficult to make a decision for quality above less "out-of-pocket" choices – but we are confident that our office will make a positive difference in your oral healthcare and even possibly your overall healthier lifestyle.

Financial Policy 

The financial policy in our office is that payment is due in full the day that service is provided (or started in the case of a multiple-visit procedure), unless prior arrangements have been made.

 

For certain lengthy appointments, a deposit in advance to reserve the doctor's time is standard protocol.

It is our goal to apprise you of the fee(s) for your treatment in advance. However, due to the very nature of any individual dental situation, your necessary treatment may differ from the original plan. We will keep you informed if any change in your treatment is recommended. You will be advised of any difference in fee(s) accordingly.

For your convenience, we accept: MasterCard, VISA, Discover, American Express, Care Credit, personal checks and cash.

CareCredit is a flexible patient/client payment program, specifically designed for healthcare expenses. CareCredit is ideal for co-payments, deductibles, treatment and procedures not covered by insurance, and can be used at over 35,000 practices nationwide.
